Consolidated Appropriations Act Input Form

Enacted in December 2020, the Consolidated Appropriations Act, 2021 (CAA-21) imposed prescription drug and health care spending reporting requirements on both group health plans and health insurers. Under the CAA-21, Nippon Life Benefits must annually submit certain information on prescription drugs and health care spending, premiums, and enrollment under our medical plans to the federal government.

In order to complete our reporting obligations we are reaching out to you, the plan sponsor, to obtain the following information. We respectfully request that this information be submitted electronically by May 1, 2023 using the form below.

Requested Information:

  • The total monthly medical premium amounts paid by you, the plan sponsor, on behalf of participants and beneficiaries in calendar-year 2022; and
  • The total monthly medical premium amounts paid by participants and beneficiaries in calendar-year 2022 (that is, separate from the medical premium amounts paid by you as the plan sponsor).
