Provider Verification Requirement

A current listing of the participating Hospitals, Physicians, and other providers is available through an on-line Preferred Provider directory. By accessing the Nippon Life Benefits website (www.nipponlifebenefits.com) you can review Preferred Provider directories for your PPO Network. Select a Directory under Find a Provider on the Welcome to Nippon Life Benefits home page and then continue to follow the prompts.

If you do not have internet access, you may request a paper copy of the provider directory for your PPO medical network by calling Nippon Life Benefits at 1-800-937-6542. Whether using the internet or a paper directory, we recommend that you (1) verify your provider's participation in the network before seeking treatment and (2) confirm PPO participation with your provider when making your appointment.

You may be responsible for payment of all or part of the fees for those professional services that are not paid or covered by your health benefit plan.
